COVID-19 Treatment Consent Form
To our wonderful clients, we want to sincerely thank you for your patience and understanding during this unprecedented time. The health, safety and well being of our clients, staff and community are most important to us.
In accordance with the CDC recommendations and Illinois State Mask Guidance, as of February 28, 2022 face masks are no longer required indoors regardless of vaccination status. Therefore, face masks are optional for clients and estheticians. If you prefer your esthetician wear a face mask, please let us know and we will kindly accommodate.
Here are a few of many safety protocols Sisters has established to help us navigate the transition to the “new normal.”
-We are following all the CDC and IDPH guidelines.
-Sanitation stations at entry to be used upon arrival and available for use in each treatment room.
-As always, all tools, stations, equipment and contact surfaces will be sanitized and/or disinfected between each client.

By filling out the form below and submitting, you agree to comply with the safety protocols above and are that you are at the salon at your own risk, releasing Sisters Skin Care & Waxing Salon from any liability related to COVID-19. For the safety of our customers and staff, failure to comply with these written instructions or verbal instructions from staff may result in your removal from the premises.
